(we) hereby authorize Touched by Toy LLC. dba SHADDAI Salon & Suites ("SHADDAI") to initiate credit Entries and to initiate, if necessary, debit Entries and adjustments for any credit Entries in error, to my/our Checking/Savings account indicated below and further authorize the financial institution listed below to debit and/or credit the same to such account. This authority is to remain in full force and effect until SHADDAI Salon & Suites has received written notification to terminate authorization. I understand and agree that my account will be drafted on or about the 1st day of the week for the current week's installment and any additional fees agreed to in the License Agreement.
